Job summary

St. Vincent de Paul CARES in Leesburg, FL is seeking a Case Manager to provide assessment, planning, linkage and advocacy for individuals and families enrolled in rapid rehousing services so they may access permanent housing and achieve housing stability.

The role requires a clear understanding of the service delivery system, strong documentation, and dedication to the PQI and Accreditation processes while working closely with the Coordinated Entry System and community partners.

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum 2 years’ experience serving homeless or at-risk families/individuals.
• Experience with wrap-around services and CoC providers.
• Proficient with Excel, PowerPoint, and Outlook.
• Knowledge of RRH standards (HPRP, ESG, CoC, etc.).

Responsibilities

• Screen eligibility and document findings.
• Assess individuals and family members to develop housing goals.
• Develop Housing Stability Plans addressing crisis, barriers, and move-in.
• Monitor progress and maintain accurate documentation.
• Coordinate with the Coordinated Entry System and local providers.
• Provide referrals and access to community resources.
• Attend RRH meetings and adhere to PQI policies.
• Uphold high ethical standards and mission alignment.
