Job summary

RHA Health Services is seeking a Qualified Professional for our team in North Carolina. You will coordinate interdisciplinary plans and ISPs, interview individuals to identify needs, and monitor progress toward health, clinical and functional outcomes.

You will oversee residential activities, ensure safety and community integration, and collaborate with leadership to enhance procedures and workflows. Requires a social services degree and driver’s license.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities; 1-year if paired with Master’s degree.
• Valid driver’s license, auto insurance, and reliable transportation.
• Supervisory/management experience and experience writing person-centered (individualized service) plans preferred.
• Must be at least 18 years of age.

Responsibilities

• Coordinate IDT meetings and the ISPs.
• Interview and identify service needs with individuals through direct interviews.
• Supervise and coordinate person-centered plans and monitor outcomes.
• Oversee residential activity, home appearance, leisure materials, cleanliness and community integration.
• Report to the Administrator and collaborate to improve procedures and workflows.
