Job summary

Cappella Grand Junction is seeking a part-time QMAP Care Partner for Sunday through Tuesday, 7:00 AM to 3:30 PM. The role includes assisting residents with daily personal care, supervising medications under nursing supervision, and maintaining accurate MAR records.

Requirements include QMAP-A/L or equivalent certification, high school diploma or GED, and 1 year elder care experience preferred. The position offers a competitive wage of $20 per hour with benefits and equal opportunity employment.

Qualifications

• Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation required.
• Must be able to read, write and speak English.
• High school or GED required.
• One year elder care experience preferred.
• Strong self-management and organization skills.
• Strong multi-tasking and adaptability; able to handle changing priorities.
• Ability to maintain confidentiality in sensitive situations.

Responsibilities

• Meet daily personal care needs of residents in alignment with mission and values.
• Supervise medications for residents under licensed nurse supervision.
• Review MAR and administer medications to correct resident, dose, time, medication and route.
• Assist residents with ADLs, including bathing, dressing, meals and routines.
• Anticipate and report resident care needs to supervisor.
• Maintain open communication with supervisor and peers about resident issues.
• Monitor and document resident condition; restock supplies as needed.
• Attend monthly staff meetings and perform other duties as assigned.
