Job summary

Family-Residences-and-Essential-Enterprises,-Inc in Town of Hempstead, NY is seeking a Qualified Intellectual Disabilities Professional (QIDP) to develop, implement, and oversee Comprehensive Functional Assessment plans and active treatment calendars for individuals with IDD.

Qualifications

• Bachelor's degree in related field required.
• Minimum 1 year experience supporting IDD individuals and/or complex medical needs.
• Minimum 1 year auditing/compliance experience.
• SCIP and CPR/FA training required (provided on hire).

Responsibilities

• Coordinates services and monitors active treatment and programming for IDD individuals.
• Creates and implements comprehensive functional assessment plans and activity plans with learning goals.
• Develops and maintains active treatment calendars and trains staff in their use.
• Leads interdisciplinary team (IDT) meetings and integrates services from team members.
• Documents client progress with timely notes and program reports for management, ensuring regulatory compliance.
• Oversees service provision per individualized plans to ensure quality of care.
• Provides training and guidance on client goals, behavior support plans, and procedures.
• Acts as liaison with families and advocates, reporting progress and addressing concerns.
• Ensures client rights are protected and needed services are advocated for.
