Job summary

AbleLight in Littleton, CO is hiring a QDDP Case Manager to work with people supported and their team to implement individualized support plans reflecting desired outcomes. You will lead staff in plan implementation in compliance with state and federal regulations.

Responsibilities include facilitating assessments, coordinating services, and ensuring proper documentation while promoting dignity and self-worth of individuals served.

Qualifications

• Bachelor's degree in human services or a medical field is required.
• Four years of case management experience.
• Current DDP Certificate from the National Association of Qualified Intellectual Disability Professionals (QIDP).
• Valid driver's license.
• Must be able to pass a background check.

Responsibilities

• Facilitates planning, development, and implementation of person-centered plans with assessments and data review.
• Assists people with Intellectual Disabilities in accessing needed medical, social, educational, and other services.
• Provides or coordinates direct case management activities for adults with Intellectual Disabilities.
• Completes documentation for maintenance of the case management record and for billing purposes.
