Job summary

Tufts Medical Center, Inc. is seeking a Respiratory Therapy Navigator to support pulmonary patients across the continuum of care. You will work with physicians, nurses, case managers, and other staff to educate patients, coordinate discharge planning, and follow up to prevent readmissions.

The role emphasizes education, care coordination, and collaboration with a multidisciplinary team to optimize patient outcomes in pulmonary disease management.

Qualifications

• Associate’s degree from an accredited Respiratory Care program.
• Massachusetts Respiratory Care Licensure.
• Registered Respiratory Therapist (RRT).
• Basic Life Support (BLS) Certification.
• Pulmonary Disease Educator Certification OR Obtain within six (6) months of hire.
• Three (3) years of experience as a Registered Respiratory Therapist.
• Preferred: Bachelor’s degree in Respiratory Care or Health Sciences.

Responsibilities

• Evaluate patients admitted with COPD, COPD Exacerbation, Respiratory Failure, and Asthma for education needs.
• Assist continuity of care with respiratory orders for discharge.
• Collaborate with care team to plan education on COPD, smoking cessation, medications, oxygen, and rehab.
• Provide patient education materials tailored to literacy levels and needs.
• Make patient follow-up calls.
• Maintain COPD/ Asthma statistics and readmission data for quality improvement.
• Participate in the continuous quality improvement process.
