Job summary

Northwestern Medicine is seeking a COPD Navigator Respiratory Therapist for full-time day shifts. The role provides COPD education, inhaler technique coaching, oxygen use guidance, and transition planning for patients, with follow-up calls and remote monitoring as needed.

The candidate will collaborate with providers, nursing, case management, and pulmonary rehabilitation to coordinate care, document interactions, and support efforts to reduce readmissions.

Qualifications

• Graduate of an Accredited Respiratory Therapy Program.
• Certified Respiratory Therapist (CRT) or Registered Respiratory Therapist (RRT).
• Active or Pending Illinois Respiratory Care License.
• Basic Life Support (BLS) required or in progress.

Responsibilities

• Provide patient education on COPD, inhaler technique, oxygen use and care plan.
• Conduct follow-up calls and participate in remote monitoring.
• Collaborate with providers, nursing, case management and pulmonary rehab to coordinate care.
• Document patient interactions and support efforts to reduce readmissions.
• Precept students and act as a positive team role model.
