Job summary

Northwestern Medicine is seeking a Respiratory Care Practitioner I to function as a COPD navigator, providing individualized inpatient education on COPD, inhaler technique, prescribed oxygen use, and symptom recognition. The role includes follow-up calls, remote monitoring support, and collaboration with the care team to coordinate care and reduce readmissions.

The position requires CRT or RRT, active or pending IL license, BLS, and completion of an accredited Respiratory Therapy program.

Qualifications

• Graduate of an Accredited Respiratory Therapy Program.
• Certified Respiratory Therapist (CRT) or Registered Respiratory Therapist (RRT).
• Active or Pending Illinois Respiratory Care License.
• Basic Life Support (BLS).

Responsibilities

• Provide inpatient education on COPD, inhaler technique, oxygen use, and symptom recognition.
• Coordinate care with providers, nursing, case management, and pulmonary rehabilitation.
• Document interactions and contribute to outcomes improvement and readmission reduction.
• Support remote patient monitoring by reviewing symptoms and vitals per protocols.
• Precept students and serve as a positive role model for teamwork.
