Job summary

Tufts Medicine is seeking a Pulmonary Disease Navigator to support patients with respiratory conditions through education, care coordination, and discharge planning. You will collaborate with physicians, nurses, respiratory therapists, case managers, and social workers to guide patients from admission to home and ensure they understand their treatment plan.

Minimum qualifications include an Associate’s degree in Respiratory Care, Massachusetts licensure, RRT, and BLS, with three years as an RRT;

Qualifications

• This Associate’s degree is required for the role.
• Massachusetts Respiratory Care Licensure is required.
• Registered Respiratory Therapist (RRT) certification is required.
• Basic Life Support (BLS) certification is required.
• Pulmonary Disease Educator Certification or obtain within six months of hire is required.
• Three years of experience as an RRT is required.
• Bachelor’s degree in Respiratory Care or related field is preferred.

Responsibilities

• Evaluates patients admitted with COPD, COPD Exacerbation, Respiratory Failure to determine education needs.
• Assists Continuity of Care with Respiratory orders for discharge.
• Collaborates with care team to provide a plan of care including COPD education and pulmonary rehabilitation.
• Provides patient education materials matching education level and needs.
• Performs patient follow up calls.
• Maintains COPD and Asthma statistics and supports quality improvement.
