Job summary

The State of Georgia seeks a Case Manager - Registered Nurse to join the Macon-Bibb County Health Department. You will provide nurse case management for TB, STDs, and breast/cervical cancer screening follow-up, coordinating care and ensuring timely treatment across the continuum.

Responsibilities include patient education, charting in EMR, and collaboration with physicians to improve access to services, plus QA/QI and outreach activities to enhance preventive health.

Qualifications

Associate degree in nursing and one year of nursing experience with PHN coursework or equivalent.
Current Georgia RN license and CPR certification required.
Knowledge of DPH nursing protocols and public health reporting.

Responsibilities

Provide comprehensive nurse case management for TB, STD, and women's health follow-up.
Coordinate care, monitor treatment adherence, and educate patients.
Document assessments, treatments, referrals in EMR and public health systems.
Collaborate with physicians and teams to ensure continuity of care.
Engage in community outreach and QA/QI activities.
