State of Georgia is seeking a Case Manager - Registered Nurse to join the Macon-Bibb County Health Department. This role focuses on comprehensive nursing case management, coordinating care, and ensuring follow-up for TB, STDs, and breast/cervical cancer related care.
The position requires an Associate degree in nursing, Georgia RN license, CPR certification, and at least 1 year of nursing experience or PHN level experience. Public health teamwork and ongoing training are emphasized.
Formación
- Associate degree in nursing required.
- Current Georgia RN license and CPR certification required.
- PHN course completed within one year of hire preferred.
- One year nursing experience required or equivalent PHRN/PHN level experience.
Responsabilidades
- Provide comprehensive nursing case management and coordinate patient care.
- Monitor treatment adherence, facilitate access to services, and ensure timely follow-up for TB, STDs, and breast/cervical cancer screening patients.
- Document assessments, treatments, referrals, education, and follow-up in EMR systems.
- Collaborate with physicians and other providers to ensure continuity of care.
