Job summary

Georgia Department of Public Health in Macon-Bibb County seeks a Case Manager - Registered Nurse to coordinate comprehensive nursing case management for TB, STDs, and women's health follow-up. You will assess, educate, monitor treatment, and ensure timely referrals within state public health protocols.

The role requires an Associate degree in nursing, GA RN license, CPR certification, and at least one year of nursing experience. A PHN course within a year is preferred.

Qualifications

• Associate degree in nursing from an accredited college or university with current GA RN license and CPR certification.
• 1 year nursing experience and completion of PHN course within one year of hire.
• Knowledge of state public health nursing protocols is preferred.

Responsibilities

• Provide comprehensive nursing case management for patients at risk or diagnosed with TB, STDs, and breast/cervical cancer screening follow-up.
• Coordinate and monitor follow-up care, referrals, and treatment, ensuring timely completion of advised care.
• Deliver direct patient care per GA DPH nurse protocols with sound clinical judgment.
• Develop individualized care plans, monitor progress, and liaise with physicians and other providers.
• Educate patients and families on disease processes, treatment plans, medications, and healthy practices.
• Maintain accurate documentation in EMR and public health reporting systems.
