Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants in resident care and activities.
• Communicate with physicians and families about progress and discharge planning.
• Complete timely documentation in accordance with facility and payer requirements.
• Maintain safe treatment environment per infection control protocols.
• Educate residents and families on home exercise programs and safety techniques.
• Recommend equipment to support resident independence.
