Job summary

Outcome HC is seeking a licensed Physical Therapist Assistant to support licensed Physical Therapists in a skilled nursing facility. This full-time role focuses on improving residents' mobility, strength, and independence through therapeutic interventions and collaboration with the care team.

The PTA will document progress, assist with resident assessments, and help implement treatment plans, including gait training and modalities.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state PTA license in good standing.
• Experience with older adults and complex medical needs.
• Ability to move frequently and assist residents safely.

Responsibilities

• Provide physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.
• Assist with resident assessments (range of motion, strength, functional mobility).
• Implement treatment plans that may include gait training, balance activities, strengthening, and modalities.
• Observe and report changes in resident condition to the Physical Therapist and nursing staff.
• Accurately document treatments, resident responses, and progress in accordance with facility policies.
