Job summary

Three-Meadows-Post-Acute seeks a Staff Physical Therapist Assistant to assist the supervising Physical Therapist with resident care and treatment plans. You will document progress and ensure accuracy of records while maintaining a safe environment and infection control practices.

Responsibilities include educating residents and families on exercises, fall prevention, and safety. Maintain licensure, participate in team conferences, and support program development per facility guidelines.

Qualifications

• Associate degree from an accredited PTA program.
• Proficient in Net Health and Point Click Care.
• Licensed as a Physical Therapist Assistant in the state.
• Maintain state licensure and CE requirements.
• CPR certification if required by licensure.

Responsibilities

• Assist the Physical Therapist in resident care and treatment.
• Complete timely, accurate documentation per facility and payer requirements.
• Maintain a safe treatment environment and follow infection control protocols.
• Educate residents and families on home exercise programs and safety strategies.
• Communicate progress, goals, and discharge planning with the team.
