Job summary

Evolve Therapy Services is seeking a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility in Phenix City, AL. This part-time/PRN position supports resident therapy under the supervision of a Physical Therapist and participates in screenings, treatment planning, and education.

Responsibilities include individual and group therapy, modality selection, scheduling coordination, and CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Maintain licensure and certification through ongoing education.

Responsibilities

• Provide PT services under the direct supervision of a Physical Therapist.
• Conduct screenings of residents at regular intervals.
• Assist in evaluations as directed by the Physical Therapist.
• Assist in identifying goals and treatment plans.
• Deliver individual and group treatments per the treatment plan.
• Assist with selection of techniques/media and adapt as needed.
• Monitor resident responses and consult the Physical Therapist for modifications.
• Assist in schedules with the PT and team members.
• Adhere to department and resident schedules; adjust as needed.
• Educate residents, families and staff on therapy use of equipment.
• Participate in education for residents and families.
• Demonstrate sound judgment in evaluation and planning of therapy programs.
• Report to work on time and coordinate shifts for productivity.
• Support Quality Assurance activities as required.
• Document CMS-compliant and medically necessary notes in NetHealth EMR.
