Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Birmingham, AL. Full Time, Part Time or PRN positions available. Assist the Physical Therapist in delivering patient-centered care, conducting screenings, and supporting treatment plans following CMS and NetHealth EMR guidelines.

Qualifications

• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist in evaluation and treatment planning with goals and modalities.
• Participate in resident, family, and staff education.
• Complete CMS-mandated documentation using NetHealth EMR.
