Job summary

Evolve Therapy Services in Phenix City, AL is seeking a Part-Time/PRN Physical Therapy Assistant to provide in-house skilled nursing facility services under the supervision of a Physical Therapist.

You will conduct screenings, assist in evaluation and treatment planning, deliver individual and group sessions, document in NetHealth EMR, and participate in resident, family, and staff education to maintain CMS compliance.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals.
• Assist in evaluations and treatment planning under supervision.
• Develop target lists with short- and long-term goals and treatment plans.
• Deliver individual and group therapy addressing strength, mobility, balance, gait, ROM and modalities.
• Monitor residents' responses and consult the Physical Therapist for modifications.
• Coordinate schedules with the therapy team to maximize productivity.
• Complete CMS-compliant documentation in NetHealth EMR.
• Educate residents, families, and staff on therapy programs.

Education
