Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Butler, AL. The PRN position supports the Physical Therapist in delivering patient-centered therapy services and ensuring quality resident care.

Responsibilities include screenings, treatment planning, group and individual sessions, and documentation in NetHealth EMR. Requires an Associate's degree in PT and an active PTA license with ongoing continuing education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in developing and following treatment plans and goals.
• Deliver individual and group treatment focusing on strength, mobility, balance and gait.
• Monitor resident response and adjust plans in consultation with the Physical Therapist.
• Assist in establishing resident schedules with the Physical Therapist and team.
