Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to join our in-house team at the Skilled Nursing Facility in Phenix City, AL. This part-time/PRN role focuses on delivering high-quality therapy services under the supervision of a Physical Therapist, contributing to resident mobility and comfort.

The ideal candidate holds an Associate’s degree in physical therapy, an active PTA license, and a commitment to ongoing professional development to meet CMS and state

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsabilidades

• Provide physical therapy services to residents under direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and in developing treatment plans with the Physical Therapist.
• Deliver individual and group treatment focusing on mobility, strength, balance, and gait.
• Document compliant and medically necessary care in CMS-compliant EMR systems.
• Coordinate resident schedules with the Physical Therapist and care team.
