Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Butler, AL. This PRN position supports the Physical Therapist and care team to deliver resident-centered therapy services on-site.

The ideal candidate will hold an Associate's degree in physical therapy and an active PTA license in the state. Ongoing professional development and CMS-compliant documentation in NetHealth EMR are required.

Qualifications

• Active Physical Therapist Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Ability to maintain CMS-compliant documentation in NetHealth EMR.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluating residents under the direction of the Physical Therapist.
• Develop treatment plans with target goals and timelines.
• Carry out individual and group treatments focusing on strength, mobility, balance, gait, and ROM.
