Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our in-house Skilled Nursing Facility partner in Humboldt, TN. This PRN role supports the Physical Therapist in delivering resident-centered care and contributes to treatment plans and documentation.

You will screen residents, assist with evaluations, and help implement therapy goals, schedules, and education for residents, families, and staff, while ensuring CMS-compliant NetHealth EMR entries.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Commitment to professional development.

Responsibilities

• Provide PT services under supervision of Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and treatment planning with PT.
• Develop resident schedules with PT and team members.
• Educate residents, families, and staff.
• Document CMS-compliant in NetHealth EMR.
• Participate in Quality Assurance programs.
