Job summary

Dublin Post Acute Dublin SNF Healthcare, LLC is seeking a Staff Physical Therapist Assistant to assist the Physical Therapist with resident care and related duties. You will support treatment plans, documentation, and safety protocols in a licensed facility.

The role involves collaborative communication with the PT, nursing, and interdisciplinary teams, participation in care conferences, and training Restorative Aides while maintaining confidentiality and compliance with state regulations.

Qualifications

• Associate Degree from an accredited PTA program.
• Proficient in computer skills such as inputting data into programs such as Net Health and Point Click Care.
• Language Skills: read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers, coworkers and families.
• Mathematical Skills: apply fractions, percentages, ratios and proportions.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Complete timely, accurate, and compliant documentation per requirements.
• Maintain a safe treatment environment following infection control and safety protocols.
• Participate in Resident Care conferences and Rehabilitation meetings as needed.
• Comply with California Laws and Regulations for PTAs per Manual Article 4 § 1398.44.
• Assist nursing with training Restorative Aides.
• Communicate with supervising PT to update physicians and staff about progress and discharge planning.
• Educate residents and families on treatment goals, home programs, and safety.
