Sunnyvale Post Acute is seeking a qualified Physical Therapist Assistant to join our rehab team in Sunnyvale, CA. The PTA will support therapists with patient care, record treatment notes, and help maintain the therapy area.

Qualifications

• Must hold an active Physical Therapist Assistant license.

Responsibilities

• Record daily treatment notes and weekly progress notes.
• Assist in maintaining therapy department and equipment.
• Provide care per PT treatment plan and coordinate with health team.
• Instruct families and nursing staff in maintenance and caregiver training.
• Participate in in-services and conferences on patient progress.
