Job summary

Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking compassionate Physical Therapy Assistants (PTAs) to join our rehabilitation team. Under a licensed Physical Therapist, you will help residents regain mobility and independence through targeted therapy and education.

Qualifications

• Current California PTA license in good standing.
• Experience in skilled nursing or post-acute rehab preferred, but not required.
• Commitment to delivering compassionate, resident-centered care.

Responsibilities

• Implement therapy treatment plans as directed by the Physical Therapist.
• Provide therapeutic exercises, gait training, and functional mobility interventions.
• Accurately document treatments in compliance with facility and regulatory standards.
• Communicate resident progress, challenges, and recommendations to the therapy team.
• Participate in interdisciplinary care planning.
• Educate residents, families, and caregivers on therapy programs and outcomes.
• Maintain a safe, clean, and supportive therapy environment.
