Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking compassionate Physical Therapy Assistants (PTAs) to join our rehabilitation department. We are a resident-centered post-acute community dedicated to quality therapy outcomes and a collaborative team culture.

Under supervision of a licensed Physical Therapist, PTAs deliver targeted interventions to help residents regain mobility and independence. We are hiring Full-Time, Part-Time, and PRN roles at $46-$51 per hour with comprehensive

Qualifications

• Current California PTA license in good standing.
• Excellent communication and teamwork abilities.
• Experience in skilled nursing or post-acute rehab preferred but not required.

Responsibilities

• Implement therapy treatment plans as directed by the Physical Therapist.
• Provide therapeutic exercises, gait training, and functional mobility interventions.
• Accurately document treatments in compliance with facility and regulatory standards.
• Communicate resident progress, challenges, and recommendations to the therapy team.
• Participate in interdisciplinary care planning.
• Educate residents, families, and caregivers on therapy programs and outcomes.
• Maintain a safe, clean, and supportive therapy environment.

As a resident-centered skilled nursing and post-acute community, we are dedicated to delivering high-quality therapy outcomes, fostering strong communication, and maintaining a supportive team culture.

We are currently hiring for Full-Time, Part-Time, and PRN opportunities.

Position Overview

As a Physical Therapy Assistant, you will work under the supervision of a licensed Physical Therapist to deliver targeted therapeutic interventions. Your role will be essential in helping residents regain mobility, build strength, and achieve maximum independence-directly impacting their quality of life and recovery outcomes.

Key Responsibilities

• Implement therapy treatment plans as directed by the Physical Therapist
