Job summary

Sunnyvale Post Acute is seeking a Physical Therapist Assistant (PTA) to join our rehab team in Sunnyvale, CA. We offer full-time, part-time and PRN opportunities and a competitive hourly rate of $40–$55.

New grads welcome. Responsibilities include treating patients under the PT, documenting notes, assisting in maintaining the department, and supporting discharge planning and caregiver training.

Qualifications

• Valid Physical Therapist Assistant license is required.
• New grads welcome.

Responsibilities

• Treat patients as directed by the Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department.
• Participate in patient care and rehabilitation conferences as needed.
• Assist with cleaning and maintenance of treatment area.
• Follow physician treatment plans for patient care.
• Assist nursing with training Restorative Aides.
• Communicate patient progress and plans with supervisor and health team.
• Instruct families or staff in maintenance and caregiver training for discharge.
• Report equipment problems and ensure good working order.

Job Responsibilities

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department.
• Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in-services training program for other staff in the facility. Record treatment changes per policy and procedures.
• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Report any problems with department equipment so that it is maintained in good working order.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Required license or certification

• Valid Physical Therapist Assistant - License
