Job summary

Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for our Skilled Nursing Facility program in NYC and surrounding areas. You will work under the PT supervisor to deliver therapeutic services to residents in a team-based setting, following a defined Plan of Care.

We offer direct employment with our agency or the option to operate as an Independent Contractor. Compensation is hourly with weekly pay, plus benefits and flexible schedules. Licensure and EMR skills are required.

Qualifications

• Current NYS PTA License and registration required.
• EMR software proficiency required.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable) preferred.

Responsibilities

• Collaborate daily with PT Supervisor and follow the established Plan of Care for all patients.
• Provide quality therapy services to SNF residents with diverse diagnoses including TBI, CVA, Stroke, Parkinson’s, MS, cancer and orthopedic conditions.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties:

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
