Santa Cruz Post Acute is seeking a compassionate Physical Therapy Assistant (PTA) to join our rehabilitation team in a skilled nursing facility. We offer Full-Time, Part-Time, and PRN opportunities with flexible scheduling and starting pay at $45 per hour DOE plus benefits.

The PTA works under a licensed Physical Therapist to improve residents' mobility, strength, balance, and independence through personalized plans of care. Join a collaborative team committed to resident-centered care.

Qualifications

• CA PTA license required or in state at time of application.
• Graduate of an accredited PTA program.
• Excellent communication and documentation skills.
• SNF experience preferred, but new graduates encouraged to apply.

Responsibilities

• Deliver direct physical therapy treatments following individualized plans of care.
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.
• Accurately document daily treatment sessions, resident progress, and responses to therapy.
• Collaborate with the Physical Therapist and care team to support resident-centered goals.
• Educate residents and caregivers on exercise programs and mobility techniques.
• Maintain compliance with state and federal regulations and professional standards.
