Santa Cruz Post Acute is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. The PT will evaluate, plan, and provide skilled services to help residents improve strength, mobility, balance, and independence in daily activities.

We offer full-time, part-time, and PRN opportunities. Starting pay is $50 per hour (DOE) with a comprehensive benefits package and ongoing education support. California PT license required; SNF experience preferred but new grads welcome.

PRN Starting at $50-58/hour DOE + Benefits Santa Cruz Post Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our skilled rehabilitation team. We deliver patient-centered care in a supportive, collaborative setting where your expertise truly makes a difference. Full-time, part-time, and PRN opportunities are available. Position Summary The Physical Therapist is responsible for evaluating, planning, and providing skilled physical therapy services to help residents restore strength, mobility, balance, and functional independence. This role works closely with the interdisciplinary care team to ensure high-quality outcomes.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical and functional impairments.
• Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards.
• Collaborate closely with nursing staff, therapy team, physicians, and families to support resident goals.
• Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.
• Participate in interdisciplinary care conferences and contribute to overall care planning.
• Maintain compliance with state, federal, and professional practice regulations.

Qualifications

• Current Physical Therapist (PT) license in the state of California.
• Graduate of an accredited Physical Therapy program.
• Strong communication, documentation, and time-management skills.
• SNF experience preferred but not required- new graduates are welcome!
