Job summary

Santa Cruz Post Acute is seeking a compassionate Physical Therapy Assistant (PTA) to join our rehab team in Santa Cruz, CA. We offer full-time, part-time, and PRN roles with flexible scheduling.

Qualifications

• Current Physical Therapy Assistant (PTA) license in the state of California.
• Graduate of an accredited PTA program.
• SNF experience preferred, but new graduates are encouraged to apply.

Responsibilities

• Deliver direct physical therapy treatments following individualized plans of care.
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.
• Accurately document daily treatment sessions, resident progress, and responses to therapy.
• Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team.
• Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques.
• Maintain compliance with state and federal regulations and professional standards of practice.
