Job summary

Santa Cruz Post Acute is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. The PT will evaluate, plan, and provide skilled services to help residents improve strength, mobility, balance, and independence in daily activities.

We offer full-time, part-time, and PRN opportunities. Starting pay is $50 per hour (DOE) with a comprehensive benefits package and ongoing education support. California PT license required; SNF experience preferred but new grads welcome.

Qualifications

• Must hold a California PT license.
• Graduate of an accredited Physical Therapy program.
• SNF experience preferred, but new grads welcome.
• Excellent communication and documentation skills.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical impairments.
• Provide gait training, strengthening, balance activities, pain management, and mobility therapy.
• Document evaluations, daily notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy, physicians and families to support resident goals.
• Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.
• Participate in interdisciplinary care conferences and contribute to care planning.
• Maintain compliance with state, federal, and professional practice regulations.
