Job summary

Flagstar Rehab is actively seeking licensed Physical Therapists to join our growing network of Skilled Nursing Facilities in NYC and surrounding areas. You will evaluate, treat, and educate patients to improve physical function and cognition, working with a diverse multidisciplinary team.

We offer options for Direct Employment or to Be Your Own Boss as an Independent Contractor, plus weekly pay and benefits. NYS license and CPR are required, with EMR competence helping streamline documentation.

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR Competence.

Responsibilities

• Provide evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide therapy services to SNF patients with various diagnoses such as TBI, CVA, stroke, Parkinson’s, MS, cancer, and orthopedic conditions.
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free-of-charge.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
