Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant to join Evergreen Nursing & Rehab in Effingham, IL. The role involves implementing patient-specific treatment plans under supervision, and documenting progress for the supervising PT.

A PTA license and an associate degree are required, with prior PTA experience preferred. The position follows a PRN-daytime schedule at the Effingham facility, contributing to quality patient care and collaborative teamwork within the therapy

Qualifications

• Associate Degree in Physical Therapist Assistant Program required.
• Current, unencumbered state PTA license.
• 1 year of previous experience as a Physical Therapist Assistant preferred.
• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
