Continuum Therapy Partners is seeking a Physical Therapist Assistant to join our patient-centered rehab team at LaBelle Health and Rehabilitation Center in LaBelle, FL. The role involves implementing individualized treatment plans under supervision, documenting progress, and educating patients on home exercise programs.

The ideal candidate holds an Associate Degree in Physical Therapist Assistant, an active PTA license, and at least one year of experience.

Qualifications

• Must have active state Physical Therapist Assistant license.
• Associate degree completed in PTA program.
• Minimum 1 year of experience as a PTA preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

• Associate Degree in Physical Therapist Assistant Program required
