Job summary

Flagstar Rehab is seeking Physical Therapists (PT) for Skilled Nursing Facilities. The team offers flexible scheduling, a “choose the way you earn” model, and support in establishing a corporation for independent work. Remote onboarding enables a smooth transition for new roles while meeting geriatric care needs.

Pay rates range from $46-$48 per hour (W-2) and $57-$60 per hour (1099). Multiple shift options and weekly pay are provided.

Qualifications

• Current NJ PT License and Registration.
• Valid BLS Certification.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.
