Job summary

Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for Skilled Nursing Facilities. We offer a flexible earning model and quick onboarding, supporting you to select facilities that fit your lifestyle and goals.

Hourly pay ranges from $34–$36 for W-2 roles and $38–$40 for 1099 roles, with various full-time, part-time, and per diem opportunities available.

Qualifications

• Current NJS PTA license and registration is required.
• Valid BLS certification is required.
• Experience with EMR systems is preferred.
• Meets all state and federal employment requirements for working in a nursing facility.

Responsibilities

• Collaborate daily with PT Supervisor and follow Plan of Care for all patients.
• Provide quality therapy services to residents in SNF settings with geriatric diagnoses.
• Monitor and document patient progress.

Physical Therapy Assistant Requirements and Responsibilities

• Current NJS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
