Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for Skilled Nursing Facilities in New York City and surrounding areas. You will collaborate with a multidisciplinary team to deliver treatments and educate patients and their families to improve physical function and cognition.

We offer Direct Employment with our agency or as an Independent Contractor, with competitive hourly pay and weekly pay. Flexible schedules are available including full-time, part-time, per diem, and weekend

Qualifications

• Current NYS PTA license and registration.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Collaborate with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting for various conditions.
• Monitor and document patient progress with EMR software.

Physical Therapy Assistant Job Duties

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
