Job summary

Continuum Therapy Partners is seeking Physical Therapist Assistants to join our therapist-led team at Montgomery Nursing & Rehab Center in Hillsboro, IL. This PRN position supports personalized patient care under licensed PT supervision.

You will implement treatment plans, educate patients on home exercises, and document progress in the chart, contributing to a responsive therapy environment. Current PTA license required; Associate Degree in PTA preferred; 1 year experience preferred.

Qualifications

• Associate degree required.
• Current, unencumbered state PTA license.
• 1 year of PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
