Job summary

Evolve Therapy Services in Olivia, MN is hiring a compassionate Physical Therapy Assistant for our in-house Skilled Nursing Facility. This full-time role, with sign-on and tuition reimbursement options, supports the Physical Therapist in delivering resident-centered therapy services.

The position emphasizes plan development, screening, treatment implementation, family and staff education, and compliant documentation using NetHealth EMR systems.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying target lists with short- and long-term goals and treatment plans.
• Conduct both individual and group treatment as assigned and outlined in the treatment plan.
• Assist in selecting physical therapy techniques/media and adapt as indicated by resident performance.
