Job summary

Evolve Therapy Services in Olivia, MN is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. This full-time role supports the Physical Therapist, helps design and implement treatment plans, and contributes to resident wellness.

We offer a $5,000 sign-on bonus and tuition reimbursement to eligible staff. The position also welcomes part-time or PRN arrangements based on staffing needs and resident care requirements.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Screen residents periodically to determine intervention needs.
• Assist in evaluation and in developing treatment plans with the PT.
• Help establish short- and long-term goals and activities in the treatment plan.
• Conduct individual and group treatment focusing on mobility, gait, ROM, balance.
• Select techniques and adapt activities per resident performance.
• Monitor resident response and modify as directed by the PT.
• Maintain resident schedules with the PT and care team.
• Complete CMS-required documentation in NetHealth EMR.
• Participate in resident, family and staff education.
