Job summary

Evolve Therapy Services in Olivia, MN is seeking compassionate Physical Therapists for an in-house Skilled Nursing Facility to provide high-quality resident care. The role offers Full Time, Part Time, or PRN positions with a $5,000 sign-on and tuition reimbursement, plus comprehensive benefits.

You will evaluate residents, develop treatment plans, and document in NetHealth EMR while collaborating with an interdisciplinary team to ensure optimal outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
