Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant to join our in-house Skilled Nursing Facility in Olivia, MN. You will work under the direct supervision of a Physical Therapist to deliver resident-centered therapy services and participate in treatment planning.

The role requires an Associate's degree in physical therapy, an active PTA license, and ongoing licensure maintenance. Benefits include a $5,000 sign-on and tuition reimbursement, with full-time, part-time or PRN options.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluations and treatment planning under the PT's direction.
• Implement treatment plans including strength, mobility, gait, ROM and modalities.
• Maintain CMS-compliant documentation in NetHealth EMR.
• Collaborate with resident, family, and staff education.
