Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. You will provide patient-focused therapy under the supervision of a Physical Therapist and contribute to coordinated resident care.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the PT.
• Screen residents regularly to determine intervention needs.
• Assist in evaluating resident progress and developing treatment plans.
• Conduct individual and group treatments per the plan, including mobility, gait, ROM, balance, and modalities.
• Assist in selecting techniques/media and adjust as indicated by resident performance.
• Monitor resident responses and consult PT for modifications.
