Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for our Skilled Nursing Facility program in NYC. You will work with a multidisciplinary team to deliver quality therapy and patient/family education to improve function and cognition.

Requirements include a current NYS PTA license, registration, EMR proficiency, and eligibility to work in the U.S. We offer options for direct employment or independent contractor roles, weekly pay, and comprehensive benefits.

Qualifications

• Licensed PTA in NYS and registered.
• Experience collaborating in a multidisciplinary team.
• Proficient with EMR systems and documenting progress.

Responsibilities

• Collaborate daily with PT Supervisor and follow Plan of Care.
• Provide therapy services to SNF patients with varied diagnoses.
• Monitor and document patient progress using EMR software.
