Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Newark, Ohio. The PTA will work under the supervision of a Physical Therapist to provide resident-centric therapy services and contribute to treatment planning and outcomes.

The role requires an active PTA license in the practicing state, an associate degree in physical therapy, and commitment to ongoing licensure maintenance.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Experience with documentation and CMS expectations.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation under the direction of the Physical Therapist.
• Assist in developing treatment plans with short- and long-term goals.
• Conduct individual and group treatment focusing on mobility, strength, gait, range of motion, and modalities.
• Monitor resident responses and adjust as directed by the Physical Therapist.
• Support resident schedules and educate residents and families as needed.
• Complete compliant documentation in NetHealth EMR as required.
