Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant to support residents at an in-house Skilled Nursing Facility in Ohio. You will work under the supervision of a Physical Therapist to deliver targeted therapy services and help improve mobility, strength, and functional independence.

Responsibilities include conducting screenings, contributing to treatment planning, and documenting progress in NetHealth EMR in line with CMS guidelines.

Qualifications

• Requires Associate's degree in Physical Therapy and active PTA license.
• Ongoing continuing education to maintain licensure.
• Proven ability to work under supervision of a PT.

Responsibilities

• Provide physical therapy services to residents under supervision of a Physical Therapist.
• Conduct resident screenings to determine need for intervention.
• Assist in evaluation and treatment planning under PT direction.
• Implement treatment plans including mobility, strength, gait, balance activities.
• Document compliant and medically necessary notes in NetHealth EMR per CMS expectations.
• Participate in staff education and collaborate with the care team.
• Maintain schedules and coordinate with PT to maximize productivity.
