Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, OH. This full-time role supports the Physical Therapist by delivering direct patient care and contributing to treatment plans.

Responsibilities include screenings, group and individual therapies, documentation in NetHealth EMR, and collaboration with the care team to ensure quality resident outcomes. An active PTA license and ongoing continuing education are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluations and in developing treatment plans.
• Contribute to individual and group treatment sessions focusing on mobility, strength, balance, gait, ROM, and modalities.
• Assist in selecting techniques/media and adapting them to resident performance.
• Monitor resident response and modify interventions as directed by the Physical Therapist.
• Help establish and adjust resident schedules with the team.
• Document compliant, medically necessary progress notes per CMS expectations in NetHealth EMR.
