Job summary

Evolve Therapy Services is seeking a full-time Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Newark, OH. You will provide physical therapy services under the direct supervision of a Physical Therapist, conduct screenings, assist in evaluations, and contribute to treatment plans to improve resident mobility and independence.

Requirements include an Associate's degree in Physical Therapy and an active PTA license in state(s) of practice, with ongoing continuing

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying targets and treatment plans with short- and long-term goals.
• Conduct individual and group treatments as outlined in the plan, focusing on mobility, strength, gait, balance, and modalities.
• Assist in selecting PT techniques/media and adapting as indicated by resident performance.
• Monitor residents’ responses and coordinate with the PT for modifications.
• Assist in scheduling and coordinating resident therapy with the team.
• Train residents and families in equipment use and safety.
• Document compliant and medically necessary CMS CMS NetHealth documentation.
