Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Fostoria, OH. The role works under a Physical Therapist to deliver comprehensive rehab services to residents, including screenings, evaluations, and personalized treatment plans.

The position requires an Associate's degree in physical therapy, an active PTA license, and commitment to continuing education to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS documentation in NetHealth EMR system.

Responsibilities

• Provide physical therapy services under the supervision of the Physical Therapist.
• Conduct resident screenings to determine need for intervention/treatment.
• Assist in evaluations and in establishing short and long-term goals.
• Deliver individual and group treatments per the treatment plan.
• Assist in selecting techniques and adapting activities per resident progress.
• Document outcomes and participate in care planning with the PT and team.
• Maintain compliant CMS documentation and schedules.
